Job 012B prints blank Multi Worksite Report (MWR) forms. These forms are BLS-designed reports used for collecting employment and wage data for individual worksites or sub-units of multiple establishment employers. The MWR is a supplement to the Quarterly Contribution Report (QCR) and may be mailed to employers with the QCR, or separately. Blank MWR forms could be for special mailings to new firms or suspected multi employers that are currently classified as singles.
Job 012B prepares blank MWR forms for non-Federal multi worksite accounts that are (or will be) active in the parameter quarter. The form includes a State-specific authorization statement, contribution report form number, and the State’s return address. It can optionally print the quarter ending date and due date. However, fields that contain account number, employer names, and addresses are all blank on the form.
If the year/quarter parameter is left as spaces, the form will print without the quarter ending date, due date, and month headings, but will still print the State-specific authorization statement, contribution report form number, and return address information. If the year/quarter parameter is entered, the quarter ending date, due date, and month headings will print, as well as the State-specific information.
Job 12B uses the Job 012D parameters set in the ES2N CICS screen. The type of mailing parameter should be set to “B” for this job. No other 012D parameters are used for Job 012B. See Job ES2J012B parameters in the Related Links for additional information.
