Job ES2J012F - Print Federal Single/Multi Worksite Forms

This job executes the following programs (click to link to each program description):


Job Features and Functions

  • Prints Federal MWR forms for initial and subsequent mailings
  • Uses Job parameters, to select Fed multi’s from Micro for Fed MWR
    • Year/Qtr can be for any quarter on Micro or any future quarter
    • Option to select new and/or existing Federal multi’s
    • Option to sort forms by Zip code; default is UI account number
    • Option to print continuation forms - sorts by number of forms
    • Option to print prior quarter delinquent multi’s only
    • Produces label file for downloading-YBUscX.A145.MWR.labels, where "sc" is the State FIPS code
    • Produces a print file for release to HP printer-Avery labels 2”X4” (Not approved by BLS)

Job 012F is similar to Job 012D and produces the Federal Single/Multi-Worksite Report for Federal employers. This is the BLS-designed report used for collecting employment and wage data from Federal Government civilian employers. This form is intended to be mailed to all Federal Civilian Employers each quarter to obtain monthly employment and quarterly total wage data for each worksite.


The same MWR form is generated for both single and multi-Federal employers. For single federal employers, administrative information will be printed at the top and in the first box of the seven box form. For multi federal employers, administrative information for the parent record is printed at the top of the form, and each active worksite's administrative and physical address information is printed in the various boxes of the form.


Options exist in this job to include multi families in pre-solicitation status, to select or exclude EDI or PEO employers, and to select initial mailing or follow-up mail-outs.


For Service Center States, MWR forms can be printed on their Hewlett Packard PC printers. However, not all HP printers work for this selected option. The cause of the issue is unknown. The process was developed in Iowa and cannot be changed.


This job is essentially obsolete. Job 012P (Create File for MWR Print Contractor) and 012W (Print MWR-Web Solicitation Forms) are assuming these functions, along with Federally-collected EDI data.
