Unemployment-Insurance Address (UIA) Block Fields

Fields in the Unemployment Insurance (U-I) address block include the address type code (IMT-UI-ADDR-TYPE), the mailing code (IMT-MAIL-CODE), the attention name (now officially referred to as the “contact” name - IMT-ATTN-NAME), the first and second U-I street address lines (IMT-UI-ADDRESS and IMT-UI-ADDRESS2, respectively), the UI city name and state postal abbreviation (IMT-UI-CITY and IMT-UI-STATE, respectively), and the 9-digit U-I zip code (IMT-UI-ZIP).
